| Title | Capitalize all words |
| Author(s) | List primary author first. List all names last name first with up to 2 initials with no spaces |
| Presenter(s) | Ensure that the following information
for the presenting author(s) is included in the submission: 1. Complete mailing address 2. Telephone with area code 3. Fax number with area code 4. E-Mail address, most important for future communication. |
| Affiliation(s) | Provide the primary lab affiliation name, city, state and zip code. If several sites were used for data collection, authors with a mutual affiliation can be listed first, followed by the mutual affiliation. |
| Abstract Body Format | Must be 300 words or less,
exclusive of title, author(s) names, and institutional affiliation(s). |
